Adding & Using Custom Questions in HRAs
Unlock HRAs include a standardized set of validated clinical questions that cannot be changed. These core questions establish risk consistently across health systems and support benchmarking.
In addition, you can add up to 10 custom questions at the end of any HRA to capture extra information relevant to your service line or organizational goals.

What are Custom Questions?
Custom questions allow you to gather workflow-specific data beyond the core clinical model. This helps you:
- Route patients more precisely across subspecialties
- Capture insurance or coverage details for coverage-based outreach
- Gather additional context for nurse navigation or triage
- Enable hyper-personalized CTAs in follow-up plans
Examples of Custom Questions
- Knee & Hip HRA – Add “Was this injury sports-related?” to route patients to sports medicine vs. orthopedic surgery
- Heart HRA – Add a vascular-specific question to guide subspecialty referrals
- Any HRA – Add an insurance coverage question to inform coverage-based outreach and resource allocation during follow-up

How to Add Custom Questions
- Log into the HRA Console.
- Navigate to the HRA you’d like to edit and click Customize HRA.
- Scroll to the Custom Questions module.
- Click Add Custom Question.
- Select the question type (for example: multiple choice, checkbox, text field).
- Enter your question wording and configure logic if needed.
- Save and publish.
Connecting Custom Questions to Follow-ups
Custom questions can also trigger follow-up actions:
- Map responses to specific CTAs in the Follow-Up Plan
- Segment remarketing or nurture emails based on participant answers
- Capture additional fields for export in raw data reports

Reporting and Data Access
Responses to custom questions are included in:
- Raw data reports (exportable CSVs)
- Universal webhook payloads (if integrated)
- Performance dashboards, when mapped to custom CTAs

Best Practices
- Limit to 2-3 custom questions to avoid participant drop-off
- Focus on questions that create actionable next steps for care teams
- Use plain language for patient clarity
- Work with your Client Success Director to align custom question design with service line strategy
