A Health Risk Assessment (HRA) is a patient-facing tool that helps users self-assess symptoms, understand their risk level, and receive personalized next steps—whether that means scheduling, education, or follow-up by a care team.

Clinically Validated Risk Algorithms

Each HRA is built using a validated clinical scoring model:

For example:

  • Heart HRA: Based on the Framingham Heart Study
  • Knee & Hip HRAs: Use the Oxford Knee and Hip Scores
  • Oncology HRAs: Reference USPSTF guidelines
  • All other HRAs have a validated scoring model as well

  • Custom Questions – Up to 10 workflow-specific questions can be added at the end of the assessment (e.g., insurance type, clinic preference)
  • Risk Report Language – Customize the explanatory copy under each risk factor
  • CTA Mapping – Adjust follow-up actions shown based on risk level
